Get a note from your doctor recommending you go on a weight loss program for a specifically named ailment
Step One
Find out if you have a Health Spending Account at work. Find out who the plan administrator is. Inform your SureSlim clinic who your plan administrator is.
Step Two
When you sign up SureSlim will provide you with a letter from our doctor and your payment receipt. Those are the only 2 documents you need to make your claim.
Step Three
Fill out a claim form from your HR department or your Health Spending Account administrator and submit it with your SureSlim payment receipt and the letter from SureSlim’s doctor.
Finally your HSA can help you treat your weight and medically related issues such as: Diabetes, High cholesterol, Hypertension, Skin conditions, Cardiovascular issues.
